TL;DR

Hijacking an account didn’t require breaking a password. It was enough to ask the AI support agent to “change the email on this account.” In late May 2026, Meta disclosed that a vulnerability in Instagram’s AI-assisted account-recovery system (High Touch Support) had been abused by third parties to take over more than 20,000 accounts. The attackers had the AI run email changes and password resets without it verifying that the requester was the rightful owner of the account, and bypassed two-factor authentication too. The structural problem: the AI recovery agent performed high-impact operations before independently verifying the requester’s ownership and authorization. We analyze this through Pillar 03 (Agent Authority Proof) as a structure in which the agent’s action is decoupled from the proof of authorization, framed as a division of labor with detection. Incident overview

  • Subject: Instagram’s High Touch Support (HTS), an AI-assisted account-recovery system
  • Discovery: On 2026-05-31, Meta discovered the HTS vulnerability. Abused by third parties, 20,225 Instagram accounts were taken over
  • Where the flaw was: HTS did not verify that the email address supplied with the request belonged to the target account. The attacker asked the AI support to change the account’s registered email to one they controlled, obtained the password-reset link, and logged in. For accounts without two-factor authentication (2FA), this bypass led straight to takeover
  • The technique: Not a technical exploit — by simply asking the AI support, in conversation, to “change the email on the account,” the recovery operation ran without any ownership check. The attackers reportedly used a VPN to appear in the same general region as the target account before making the request
  • Notable victims: The former Obama White House account, the account of the U.S. Space Force’s Chief Master Sergeant, and short, high-resale-value usernames were among the targets
  • Likely impact: Reachability of contact information (email / phone), date of birth, posts and stories, direct messages, and account activity
  • The crux: The AI recovery agent performed high-impact operations (email change, password reset) before independently verifying the requester’s authorization as the rightful owner of the account

Timeline

  • 2026-03: Meta introduces HTS (the AI-assisted account-recovery system)
  • 2026-05-31: Meta discovers the HTS vulnerability; confirms third-party abuse and account takeovers
  • 2026-06-01 to -08: TechCrunch, 404 Media, Help Net Security, and others report. The 20,225 figure, notable accounts, and the 2FA-bypass technique come to light. Meta files a breach notification with Maine’s Office of the Attorney General (06-08)
  • Response: Meta disables the AI-assisted support tool, invalidates reset links generated through the vulnerable workflow, requires additional authentication for affected accounts, and instructs users to reset passwords

Note: The figure (20,225) and the technique are based on Meta’s account and reporting. The exhaustive scope of the attack and the detail of referenced data depend on the ongoing investigation, so we do not assert them here.


The attack path: how AI support runs a recovery without checking ownership

This incident stems from a structure in which the AI recovery agent does not independently verify the requester’s ownership and authorization before acting. The path by which the failure propagates into account takeover:

  1. A request through a legitimate channel: The attacker asks Instagram’s AI-assisted support (HTS) to change the registered email of the target account. The request itself takes the form of a legitimate support request
  2. Missing ownership verification: HTS accepts the email-change operation without independently verifying that the email supplied belongs to the target account or that the requester is the rightful owner
  3. The recovery operation runs: The registered email is changed to the attacker’s, and the password-reset link goes to the attacker. The reset hands login credentials to the attacker
  4. Bypassing 2FA: For accounts without two-factor authentication, the reset path leads straight to takeover, with no additional identity check
  5. Detection and notification: Meta detects the anomaly and notifies the targeted users. But this is an after-the-fact sequence that operates only after the recovery ran and the account was taken over

Structural analysis

This incident belongs to the identity-auth category under Pillar 03 (Agent Authority Proof). The central failure primitive is that the AI recovery agent, when performing high-impact operations (changing the registered email, resetting the password), does not independently verify before acting that the requester is the rightful owner of the account. As secondary we note agent-infrastructure (the AI support as infrastructure) and ai-decision-integrity (the AI’s decision to honor a request that lacks authorization).

The crux is the structure by which a “help” function — support — becomes an attack surface. The AI recovery agent changes registration details and resets credentials on the user’s behalf, to help them. But “a request arrived” is not proof that the requester is the rightful owner. As the requested email’s link to the account went unverified here, the proof of ownership was decoupled from the execution of the recovery operation. Account recovery is the path that should demand the strictest identity check, yet its authorization was not independently verified before the action.

This is the production-scale, consumer-platform version of Brief 047 (an AI agent performed a high-risk action and exfiltrated secrets before verifying the sender’s identity). The primitive both share is that the agent’s action is decoupled from the layer that authorizes and verifies it. It also connects to Brief 046 (the requester’s authorization is not independently verified before the response), 029 (an OAuth token not scoped to the operation), and 006 (the state of a credential/attribute is not independently verified), in that authorization and ownership pass without being bound to the scope of the action. What this case shows is the consequence of an AI support agent lacking independent ownership verification on the recovery path that demands the strictest authorization.


The gap between detection and proof

Meta’s discovery of the vulnerability, detection of the abuse, notification of targeted users, and visibility through reporting are all indispensable for grasping, containing, and preventing recurrence of the harm; this Brief does not deny that role. Identifying affected accounts, notifying users, and prompting 2FA activation are the highest-priority operational responses.

At the same time, detection does not, at the moment the AI runs the recovery operation, independently establish “is this recovery request coming from the rightful owner of the account.” The support conversation proceeds through a legitimate channel, and a single request looks normal as a support request. Because the ownership check was missing, “a request arrived” became the basis for executing the operation. What was missing is the at-action independent verification of “does this requester hold the ownership and authorization to perform a recovery operation on this account,” which is a separate track from after-the-fact anomaly detection and notification. As long as receiving a request is equated with proof of authorization, detection can only trail the takeover.

Pre-execution attestation closes this gap by inserting one step — proof of the requester’s ownership and authorization — into the recovery path. By requiring, before the AI support runs an email change or reset, that “this requester is the rightful owner of this account” as independently verifiable proof, the recovery operation is blocked up front when the proof of ownership does not hold — regardless of the request’s framing or urgency. Detecting the request’s content (the detection-style “does this request look plausible”) and the pre-execution attestation of ownership and authorization (“does this requester hold the authority to operate on this account”) are not substitutes but complements.   • Meta / Instagram: Discovered and remediated the HTS vulnerability, disabled the AI-assisted tool, invalidated reset links generated through the vulnerable path, and began notifying targeted users. The design of identity verification on the high-risk path of account recovery is now a point of debate
  • The AI-support question: How much ownership verification to require, and on which operations, for AI support agents that perform recovery and changes on a user’s behalf has surfaced as a cross-platform issue. The need to separate “receiving a request” from “proof of ownership,” and to place independent verification ahead of high-impact operations, has been re-recognized
  • Cross-industry point: In operations that grant delegated authority to AI agents, there is growing discussion of shifting the design’s center of gravity toward binding the agent’s action to independent verification of the ownership/attributes that authorize it (proof-as-auth / per-action attestation). Lemma’s analysis

Against the gap this incident exposed (the AI recovery agent’s action is decoupled from independent verification of the requester’s ownership and authorization), Lemma proposes a design that, before an agent takes a high-impact action, requires that the action be backed by ownership and authorization as independently verifiable cryptographic proof.

  • Pre-execution attestation of ownership (proof-as-auth): Before the AI support runs a registration change or reset, prove with a signature that “this requester is the rightful owner of this account.” Do not make “a request arrived” the endpoint of authorization
  • Selective disclosure of attributes: Prove ownership and identity with minimal disclosure, without sending raw personal data outside the environment — giving the recovery path exactly the assurance it needs
  • Scoped authority: Minimize the delegated authority granted to AI support per operation, and place ownership proof ahead of high-impact operations (email change, reset). Do not let operations beyond the scope of authorization succeed without proof
  • Evidence trail of actions: Bind recovery operations to the proof of the requester’s authorization/ownership and record them as evidence, so that even in later audits “under whose authorization was this operation performed” is independently verifiable

Through this, proof fixed at the moment of action functions as an independently verifiable trail for “does this recovery operation carry the rightful owner’s authorization,” before the operation runs. Detection and notification (after-the-fact detection and user notification) serve to grasp and remediate harm, while pre-execution attestation (pre-action ownership and authorization verification) serves the independent verification of recovery operations — each working complementarily.
